It is specific to the pediatric patient and includes: Consent, Emergency Triage Form, Privacy Notification, Caregiver Agreement, Therapy non-compliance form, Agency policies, Emergency Preparedness Plan, Medication Profile as well as all other forms required by rules and regulations.
