HOSPICE: Admit packet is compliant with all state, Medicare and Accreditation requirements. It is specific to the adult Hospice patient and includes: Consent, Emergency Triage Form, Notice of Election Form, Choice of Physician, Hospice is palliative and not curing care explanation, Privacy Notification, Agency policies, Emergency Preparedness Plan, Medication Profile, Proposed visit calendar as well as all other forms required by rules and regulations.
